Below you are able to schedule an In-Home Consultations or Virtual Workshops. Once you select an appointment time for a consultation, you will be prompted to complete an online intake form. Completing this secures your appointment and allows us to maximize our time together. A credit card or HSA card will be held on file and billed after your consultation has been completed. Anthem BCBS, Medical Mutual, & participating United Health Care (non-medicaid plans) members will only be billed co-pays/deductibles. OSU/Luminere members will not be billed any fees.
